Supplements and Medications: What You Need to Know About Interactions

Most people take supplements without telling their doctor. Most doctors don’t ask.

That gap creates a real risk. Supplements are natural. Natural does not mean inert. Several widely used supplements interact with common medications in ways that can reduce drug effectiveness, increase side effects, or — in some cases — cause serious harm.

This article covers the most clinically significant interactions. It isn’t designed to discourage supplementation. It’s designed to make sure you supplement safely — with full awareness of what you’re combining.

How Supplement-Drug Interactions Happen

Interactions between supplements and medications occur through several mechanisms.

Absorption interference — a supplement can bind to a drug in the gut and reduce how much reaches the bloodstream. Iron and calcium both do this with several medications.

Enzyme competition — many drugs are metabolised by liver enzymes, particularly the cytochrome P450 family. Some supplements either inhibit or induce these enzymes, causing drugs to clear from the body faster or slower than intended.

Additive effects — a supplement and a medication can both produce the same physiological effect. Combining them pushes that effect beyond the intended level. Blood thinning is the clearest example.

Opposing effects — a supplement can directly counteract a medication’s mechanism. St John’s Wort does this with multiple drug classes simultaneously.

Understanding these mechanisms helps you think through any combination — not just the ones listed here.

Omega 3 and Blood Thinners

Omega 3 fatty acids reduce platelet aggregation — the tendency of blood cells to clump and form clots. At standard doses, this is beneficial for cardiovascular health. But it becomes significant when combined with anticoagulant or antiplatelet medications.

Warfarin, aspirin, clopidogrel, and newer anticoagulants like rivaroxaban and apixaban all reduce blood clotting through their own mechanisms. Adding high-dose omega 3 supplementation on top amplifies the anticoagulant effect. This increases bleeding risk — particularly relevant before surgery or following an injury.

At doses of 1,000–2,000mg of EPA and DHA daily, the interaction risk is low for most people. At higher therapeutic doses, discuss it with your prescribing doctor first.

St John’s Wort: The Most Significant Interaction in Supplementation

St John’s Wort deserves its own section. It interacts with more medications than any other commonly used supplement — and the interactions are serious.

St John’s Wort powerfully induces cytochrome P450 enzymes and a drug transporter called P-glycoprotein. This accelerates the clearance of many drugs from the body, reducing their blood levels and their effectiveness.

Medications significantly affected include:

  • Oral contraceptives — St John’s Wort reduces their effectiveness, with documented cases of unintended pregnancy
  • Antidepressants (SSRIs) — combining St John’s Wort with SSRIs risks serotonin syndrome, a potentially dangerous excess of serotonin activity
  • Warfarin and anticoagulants — reduced drug levels increase clot risk
  • Cyclosporine and immunosuppressants — used by transplant patients; reduced levels risk organ rejection
  • HIV medications and antiretrovirals — reduced effectiveness with serious clinical consequences
  • Some chemotherapy agents — reduced drug levels undermine treatment

The NHS explicitly warns against combining St John’s Wort with any of these medication classes. If you take any prescription medication, avoid St John’s Wort entirely unless your doctor specifically approves it.

Iron and Multiple Medications

Iron supplementation interferes with the absorption of several important medications when taken simultaneously.

Levothyroxine — the most commonly prescribed thyroid medication — absorbs significantly less effectively when taken within two hours of iron. The interaction reduces thyroid hormone levels and undermines treatment. Always take levothyroxine at least two hours before or four hours after any iron supplement.

Quinolone and tetracycline antibiotics form insoluble complexes with iron in the gut, reducing both the antibiotic’s and the iron’s absorption simultaneously. Space iron supplements at least two hours away from these antibiotics.

Levodopa — used in Parkinson’s disease — also shows reduced absorption when combined with iron. Keep a minimum two-hour gap between the two.

Calcium and Thyroid Medication

Calcium follows the same pattern as iron with levothyroxine. Calcium carbonate and calcium citrate both bind to levothyroxine and reduce its absorption when taken close together.

People managing hypothyroidism and supplementing with calcium — which is common, particularly in older women managing both bone health and thyroid function — need to maintain a minimum four-hour gap between the two supplements and their thyroid medication.

Calcium also reduces the absorption of bisphosphonates — medications used to treat osteoporosis. The instruction to take bisphosphonates on an empty stomach with water, well away from any food or supplement, exists precisely because of this interaction.

Vitamin K and Anticoagulants

Vitamin K plays a central role in blood clotting. Warfarin works by blocking Vitamin K activity — that’s its mechanism. Taking Vitamin K supplements while on warfarin directly opposes the drug’s effect and destabilises anticoagulant control.

The interaction doesn’t mean Vitamin K is dangerous for everyone. But for anyone prescribed warfarin specifically, Vitamin K supplementation requires careful medical supervision and regular INR monitoring.

Newer anticoagulants like rivaroxaban and apixaban don’t operate through the Vitamin K pathway in the same way — but always confirm with your prescribing doctor before supplementing.

Magnesium and Certain Antibiotics

Magnesium — like calcium and iron — can reduce the absorption of quinolone and tetracycline antibiotics by forming complexes in the gut. The practical fix is straightforward: take magnesium at least two hours before or after any course of these antibiotics.

Magnesium also has mild blood pressure-lowering effects. For people taking antihypertensive medications, this additive effect is usually minimal — but worth monitoring if you start supplementing at higher doses.

The Most Important Rule

Tell your doctor and pharmacist about every supplement you take — including vitamins, minerals, herbal products, and protein powders. Supplement use belongs in your medical history.

Before starting any new supplement, check for interactions with your current medications. Your pharmacist can review combinations quickly and flag concerns before they become problems. This takes two minutes and removes almost all meaningful risk.

Supplementation and medication can coexist safely for the vast majority of people. The key is informed awareness — not avoidance.
